There are many disciplines participating in the plan of care for the total recovery of the patient. The skilled nurse, therapist, social worker, physician, and others are all involved to help the patient recover at home as quickly as possible.
All care is provided in a professional and caring manner. Our agency is here to help the patient recover at home, addressing all home care needs. Our multidisciplinary team can assist and teach caregivers who provide patients in-home care.
Homebound patients may qualify for services. Home Health Care is for patients who are no longer able to care for themselves and need the help of skilled medical professionals to transition from the hospital or nursing rehabilitation center to home. We are motivated to help the patient heal at home as quickly as possible with intermittent visits by our medical professionals.
